I started using IEM's long before I got into over the ear 'phones earlier this year.

I started with the Etymotic ER-4P and have moved to Westone's W4 multi-armature iem's.

For travel they are hard to beat. They are small enough to fit inside my pinnae. So I can sleep with them in.

As Rob said fit is critical. I have found the Comply tips fit me the best. I use the smallest diameter long ones and they provide almost total isolation while being comfortable enough to wear on 12 hour flights to The Far East.

Screaming child in the row behind me? No problemo. Loud and drunk people? Can't hear them.

They stay in fine on treadmills
and elliptical machines.

I would never use them running or biking - too much isolation.
